1. Visit policy change forms in the Customer Service section.

2. Log in to My Account section and visit my policies.

3. Call us at 1-800-800-9410. We're here Monday – Friday from 7am – 7 pm, MST and Saturday from 9am – 1pm MST.

These forms only change the information on California Casualty insurance policies.
Remember to also provide any name and address change information directly to your employer or association.
